Imminent Risk of Homelessness Screening

Where are they currently staying?

Hotel or motel NOT paid by Charitable/gov program Staying or living in a friend's room, apartment or house Staying or living in a family member's room, apartment or house Place you rent/own None of the above

Housing loss expected within 14 days?

Is there no subsequent residence identified?

Do they lack resources/support networks for other permanent housing?

All three conditions below must be Yes for Imminent Risk

Chronic Homelessness Screening

Has experienced 12+ consecutive months of continuous literal homelessness?

In past 3 years, literally homeless for 12+ months in 4+ separate occurrences?

Has a disability that prevents sustainable housing?

Physical, intellectual, serious mental illness, or substance use disorder

✓ Requires: (1) 12+ months homelessness history AND (2) Disability preventing sustainable housing
